
When a client's facial routine screens clean and they're still breaking out along the hairline, temples, upper back, and shoulders, the products causing it are usually the ones nobody asked about: conditioner, leave-in, styling cream, dry shampoo, body wash, body lotion. These are functionally leave-on products in the areas where they run off, and they're formulated far heavier than face products because nobody expects a conditioner to be screened for comedogenicity. The pattern is well documented: Plewig, Fulton and Kligman described pomade acne in 1970 in a study of 735 men, finding about 70% of long-term pomade users had a recognizable acneiform eruption on the forehead and temples, mostly uniform closed comedones. Every acne esthetician has this file. You audited their whole routine. Cleanser is fine, moisturizer screens clean, SPF is a mineral formula with nothing flagged, they're compliant with the actives, and they're doing everything you asked.
And they still have a band of small closed comedones across the upper forehead and temples. Or persistent congestion along the jaw and sides of the neck. Or an upper back and shoulders that won't clear no matter what you do to their face.
The instinct at that point is to assume hormones, diet, stress, or genetics — and sometimes it is. But before you escalate to internal causes, there's a category of external cause that most intake forms simply don't ask about, because it isn't skincare.
It's everything else they put on their body.
The distribution tells you where to look. This is the most useful diagnostic in the whole post, because the pattern is created by gravity and rinse-off physics rather than by anything intrinsic to the skin in those areas.
Upper forehead, temples, and along the hairline. This is the pomade acne signature. Product applied to the scalp and hair migrates onto the skin below it — through direct contact, through sweat, through heat, through sleeping on it. Plewig, Fulton and Kligman's 1970 paper documented this in detail: predominantly uniform closed comedones on the forehead and temples, and notably, the more elaborate commercial formulations produced it more frequently and more intensely than simple mineral oil or petrolatum.
The lesions are strikingly uniform in size, which is the tell — hormonal acne is polymorphic, product-driven comedones tend to look like each other.
Sides of the neck, jaw, and along the shoulders. Rinse-off path. When a client washes conditioner out of long hair, it runs down the back of the neck, over the shoulders, and across the upper back. Conditioner is designed to deposit — that's the entire point of the product — so it doesn't fully rinse away. It leaves residue exactly where you're seeing congestion.
Upper back, shoulders, and chest. The truncal pattern. Some of it is conditioner runoff, some is body wash and body lotion, and some is friction and occlusion — acne mechanica — from sports bras, backpack straps, athletic gear, and synthetic fabric trapping heat and sweat.
These frequently overlap, which is why truncal acne is stubborn.
Both cheeks near the ears, and behind the ears. Hair contact throughout the day, plus styling product transfer from hands and pillowcases.
When the distribution matches a product path rather than a hormonal pattern, you've almost certainly got an external cause. Follow the path.
The most under-suspected product in the entire routine. Conditioners are formulated to leave a conditioning film on hair — they're built to deposit, not to rinse clean. On skin, that film behaves like a leave-on occlusive.
The ingredients that show up: coconut oil (5), heavy lanolin derivatives like laneth-10 (4) and acetylated lanolin alcohol (4), cetearyl alcohol and cetyl alcohol (2, low but present in nearly every conditioner), isopropyl myristate (5) for slip, and a range of plant oils including avocado oil (4) and palm oil (4).
Leave-in conditioners and hair masks are the worse version of the same problem, because they aren't rinsed at all. A client using a leave-in cream on the hair around their face is applying a comedogenic leave-on product to their hairline every single day.
Pomades, waxes, gels, creams, curl creams, oils, heat protectants, texture sprays. This is the pomade acne category proper. Heavy waxes and occlusive oils are the point of these products — hold and shine come from occlusion.
Worth knowing that traditional petroleum-based pomades cause buildup, and that Plewig's data found the more complex commercial formulations were worse than plain mineral oil.
The added emollients and esters in a modern styling cream are frequently more comedogenic than the simple occlusive base people are afraid of.
Dry shampoo deserves a specific callout. It's applied at the roots, it sits there for a day or more, and it's designed to absorb oil and stay put. Estheticians are increasingly seeing hairline and scalp-margin congestion in clients who've replaced washing with dry shampoo.
Body products are formulated richer than face products, because nobody is worried about clogging pores on a shin. But the upper back and chest have large, dense sebaceous glands — they behave like facial skin and get treated like limb skin.
Body lotions and body butters are where cocoa butter (4), unrefined shea butter (4), coconut oil (5), and wheat germ oil (5) concentrate. A client who applies body butter after every shower is applying a comedogenic leave-on to their back daily. Body washes are lower risk given the short contact time, but heavily emollient "moisturizing" washes can leave residue on skin they never fully rinse from.
Sunscreen for the body — often a different, heavier product than their face SPF, and ethylhexyl palmitate (4) is extremely common in body sunscreens (more on that in Why Sunscreen Breaks Your Clients Out). Hand cream that ends up on the face. Lip balm at the perioral margin — Shelley documented "chap stick acne" as its own entity.
Fragrance and self-tanner on the chest and shoulders. Laundry detergent and fabric softener, which deposit residue on every shirt and pillowcase they own.
Not every one of these will matter for every client. But when the face routine is clean and the breakouts persist, this is the pool the answer is usually in.
Before you spend a consultation auditing shampoo, make sure you're looking at product-driven comedonal acne and not one of its two common mimics — because the fix is different.
Malassezia folliculitis (fungal acne). Presents as small, uniform, itchy papules and pustules, most often on the upper back and chest, sometimes the forehead and hairline. The distinguishing feature: it's monomorphic and comedones are absent.
It also fails to respond to standard acne treatment, which is usually what brings the client back frustrated. If they describe itch and you see no comedones, you're likely dealing with yeast, and the relevant ingredient screen is different — esters and fatty acids that feed malassezia rather than pore-clogging emollients. Acne mechanica. Driven by friction, pressure, heat, and occlusion rather than by ingredients. Classic in athletes, students with backpacks, and anyone in synthetic athletic wear.
Look at what's touching the skin and for how long: straps, pads, sports bras, helmet lines, chair backs. The distribution follows equipment, not product runoff.
These aren't mutually exclusive. A high school athlete in August might reasonably have all three at once — comedogenic conditioner running onto their back, friction from a sports bra or shoulder pads, and yeast thriving in the heat. Sorting out which component you're treating is what makes truncal acne feel so intractable when you skip the step.
Most intake forms ask about skincare. Very few ask about the rest, which means the information simply never arrives. Six questions to add:
Then screen the labels the same way you'd screen a moisturizer — the same discipline as the back-to-school teen product audit, applied to the products above the neck and below it. This is the step people skip, because reading a 40-ingredient conditioner label against a pore-clogging list feels absurd when you already have a stack of face products to get through. Which is fair — doing it by hand is genuinely tedious.
It's also the step that's now trivial. Start with whichever product has the longest contact time in the affected area. Leave-in and styling products before shampoo. Body lotion before body wash.
Add the cheap mechanical fixes immediately, since they cost nothing and don't confound the test: wash the face after rinsing conditioner rather than before, tie hair back at night, use a clean pillowcase, rinse hair forward so conditioner runs away from the back, shower promptly after workouts, wash the hairline specifically rather than stopping at the brow.
Give it a full cycle. Existing congestion takes weeks to clear even after the cause is removed. Set that expectation up front or the client will conclude the swap failed at week two and go back to the product they liked.

It solves the cases that make you look bad. The client whose face routine is perfect and skin isn't is the one most likely to lose faith and leave. Finding an external cause nobody else looked for is the single most credibility-building thing you can do in an acne practice.
